Down the forms, fill them out, choose the type of membership (New or Annual Renewing) (Individual or Dojo), make a check payable and amount based on the type of membership to Richard Petronelli and mail the forms to: Tuhon Richard Petronelli: 35 Spicer St. North Providence, RI 02904.
Note: All members whether Individual or Dojo type membership must complete and submit annually a Kuntao JuJitsu Federation application and the Federation Waiver every January 1st. Membership is a calendar year January 1st to December 31st every year. No member shall submit there payment past the grace period every January 7th. If payment is received at Head Quarters past January 7th, the member will have to payment as a new membership (whether Individual or Dojo). No membership is prorated.
